CHESA EVENT CONSENT FORM

CODE OF CONDUCT

All attendees, speakers and sponsors at our conference are required to agree with the following code of conduct. The executive director and/or board will enforce this code throughout the conference. We expect cooperation from all participants to help ensure a safe environment for everybody.

Our conference is dedicated to providing a harassment-free experience for everyone, regardless of gender, gender identity and expression, age, sexual orientation, disability, physical appearance,  race, ethnicity, religion (or lack
thereof).  We do not tolerate harassment of conference participants in any form. Sexual language and imagery is not appropriate for any conference venue, including discussion, workshops and/or team building activities. 

If you are being harassed, notice that someone else is being harassed or have any other concerns, please contact a member of the CHESA Board Immediately.

Everyone is expected to communicate and engage respectfully with each other and behave in a professional manner.

Conference participants violating these rules may be sanctioned or expelled from the conference and not provided a refund.

VIDEO/PHOTOGRAPH RELEASE FORM

I hereby grant the Canadian Healthcare Environmental Services Association (CHESA) the irrevocable right and permission to use photographs and/or video recordings of me on CHESA and other websites and in publications, promotional flyers, educational materials, derivative works, or for any other similar purpose without compensation to me.

I understand and agree that such photographs and/or video recordings of me, may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of the CHESA.

I hereby release, acquit and forever discharge the CHESA , its current and former trustees, agents, officers and employees of the above-named entities from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name.

This release is binding on me and my heirs, assigns and personal representatives.

SCAVENGER CHALLENGE LIABILITY RELEASE FORM

By signing the form below, I acknowledge and agree to the following:

Voluntary Participation
I am voluntarily participating in the CHESA Scavenger Challenge. I understand that participation may involve physical activity and carries a risk of injury.

Assumption of Risk
I understand and accept that there are risks associated with participating in this event, including but not limited to slips, falls, or other accidental injuries. I accept full responsibility for any and all injuries that may occur as a result of my participation.

Waiver of Liability
I hereby release and hold harmless the Canadian Healtcare Environmental Services Association (CHESA), its staff, volunteers, affiliates, and event partners from any and all liability for injuries, damages, or losses sustained during the course of the Scavenger Challenge.

Medical Acknowledgment
I affirm that I am in good physical health and able to participate in this activity. I understand that I am solely responsible for any required medical attention as a result of my participation.

Photographs and Media
I grant permission to CHESA to use photos or video footage taken of me during the event for promotional and educational purposes.

OPT-OUT / REVOCATION

If you do not wish to be photographed or recorded, please notify event staff upon arrival so that reasonable steps may be taken to accommodate your request. Note that incidental inclusion in group photography or video may not always be preventable.
